Several "handbook" size references that are portable
(but not water repellent) and very comprehensive are:
Wilderness Medicine by William Forgey, M.D.
Mountaineering Medicine by the Seattle Mountaineers
Both are available at various mountaineering/outdoor stores.
I agree with Rik's comment that training is the most
important "reference" you can take with you. A book is
useful but having the knowledge and skills obtained from a comprehensive
training program is essential.
