One Idea is used by my troop, a troop medical book kept in it's own travel
Each trip's leader is responsible to get the bag from the scoutmaster.
Each scout has a separate vinyl page holder in the book.
Each scout's parent signs a blank permission form each September,
with this form then copied.
Each scout's has a completed medical form, with the original staying
in the book, with two copies in the folder.
For summer camp, we notify those scouts who need to be updated,
a complete set of copies is delivered to the camp nurse.
Doesn't always work perfectly, but it eliminate the medical forms paperwork
