I went back to the source. Hope I got everyone's question answered.

So again, step #1, apply at your council office. If they tell you they don't
have a form for it, I would tell them to look harder because every council
should. If they handed them all out they should be able to call National and
get their hands on a stack real fast. If they really give you a hard time,
get back to me. Had to do it last time, might as well assume the role already.
Step #2, pay your fees.
No, I will not be working in the TP but will instead be in relationships and
helping a couple of friends on the side so they get some time off. And I'm
sure I'll wander by one or two of them during the time I'm there.
