I have, hanging on my wall, a place-mat sized, fully laminated sheet that
covers (in detail I find excessive) the basics. At the bottom, it says,
c1971, Florence P Kendall, Registered Parliamentarian NAP; and "available
from Design & Funding Inc, 1132 N Rolling Road, Baltimore MD 21228".
