Not yet; but you can photocopy the application cards from the BSA
publication called "Leadership Plans and Training", which is available from
your Council office or the local Scout Shop(tm).

Remember that the Cub Scout Leader card (sheet) was designed for all Cub
Scouting leaders; because of the variety of Boy and Varsity Scout (and now
Venturing) leaders, and their Commissioners, it's not impossible but hard to
place all of the information onto a single sheet for all of those leaders.
Stick around, Mary....I'm sure that in a few weeks or so, those card sheets
will find their way upon either the official BSA website or *someone else's*
