Yes, it is called a Youth and Adult transfer form. It can be found at

This is a pdf form and both sides (youth and adult are there). It takes an
adobe reader to open it. One thing you might also do, if the scout is in
between ranks and has some of the next rank finished, or part of a merit badge
finished, Have the Troop Advancement Chair make a list of the requirements,
with the dates, that are completed and then sign and date the list. That way
there is no question as to what has and hasn't been done when he gets to the
new troop. This can also be done for Community Service, etc. You should also
do this with the individual scout record. Make sure that this is completely
filled out (attendance, camping, hiking, service projects, troop activities,
troop meetings, etc.) These two forms, plus the form signed by the Troop
Advancement chair, will leave nothing to chance.
I just sent 2 different boys packing with their transfer forms and individual
scout records completely filled out from the day they entered Boy Scouts. I
received a message from one scoutmaster, that said that things were so much
easier to transfer since all of the information was written and signed.
