NO. However, it is recommended that he contact the counselor for any
badge that he has had no experience with.
For example, A Scout with an existing Stamp Collection need not start
over after he meets with his counselor, but one who wants to START a
Stamp Collection should speak with the counselor first.

Most definitely yes. Any nights since the Scout has joined
a Boy Scout Troop, where he either camped in tents he has pitched, or
slept under the stars can be counted.

That's up to the counselor, but most will not count it.

The only time limit is their 18th birthday.
