Class 1,2, and 3 medical forms

I'm getting conflicting answers from my council regarding Class 1,2, & 3
medical forms. Can someone on the list clarify this for me.
We are a cub scout pack. When we go on day hikes, etc. do accompanying
parents and siblings need a medical form?
If so, do those over 40 years old need a class 3 form?
If so, does that mean that those parents over 40 years old have to get a
physical from a doctor once a year to participate in even the most
simplest day exursions with our pack?
The form insturctions don't help much because the Class 3 states that it
is for those over 40 "in situations where a physical is required" that
insinuates that it replaces the class 2 for those over 40. The class 2
is not used for short excursions. So in any case, the instructions need
improved if anyone from national is listening!
