Eagle Reference letter (USA)

I, as a District Advancement Chairman, am looking to make some alterations
in the methods by which letters of reference are obtained for Eagle Scout
candidates in my district. I wish to present this concept at our next
District Roundtable on 4/13. I am looking for some examples of
preformatted letters with standard questions; e.g.
Please indicate how long you have known the candidate...
Please describe how you have seen him live the Scout Oath and Law in his
I found one such letter on the internet from Chief Seattle Council. It's
nice but it does not format well when I capture it from Netscape.
Do any of you have some standard forms that your troop uses that you would
not mind sharing?
My son, Ken, will be the guinea pig for this effort. He is finishing up
his project report. He will be the second of my sons to get Eagle and the
second of our 4 generations of Eagles which started in 1915 with my
grandfather. (My oldest son earned his in 94. Ken will be the 16th Eagle
in the family. And, yes, I saw the "Family Traditions" section in the last
EagleLetter from NESA. Hopefully I'll have our family in the next issue.)
