The product you want is Adobe Acrobat 3.0 ( per Robert Haar).
This suite contains Adobe Distiller enabling you to convert word
processing documents to pdf files read with Adobe Reader by others,
Adobe Capture enabling you to alter material scanned in Exchange, and
Adobe Exchange which allows you to create a pdf document, scan a form,
alter, and create a fill-in form from the new pdf document OR an
existing pdf document. Yes, it is powerful. I have had great fun with
it. See my example at
http://www.bsa.net/la/t230/chitadv.htm and then to Merit Badge Counselor
Application Information form No. 34405
http://www.bsa.net/la/t230/mdappl2.pdf which can be then printed,
printed and faxed, or e-mailed (attached file).
Yes, it is expensive $200 plus. However, a college student can purchase
the Academic version at most college bookstores for $49.95 plus tax.
