Patrol Adult Advisor?

In the discussion about signing off of requirements, I've seen several
references to the Adult Advisor to the patrol. This sparked my interest,
since I'm not familiar with this troop position.
I would think that such a position could cause some major problems in a
well run (read that BOY RUN) troop, since it would create a fractured
command structure. I could see where the patrol would simply ask the
advisor rather than using the patrol method.
I'd be curious on hearing how units are using this and maintain a true boy
