SM conf / B of R -- request for info

Looking for someone to please write up a 'summary' which describes the SM conf
and B of R...type of info I am interested in includes things like:
What is supposed to happen during the SM conf and B of R? Are there some
things "unique" to each? Are there some things that are "common" to each?
What are the goals for the SM conf and B of R? What key items should be
covered in each?
Who sits in on the SM conf and B of R? Is it allowable for the SM to
designate ASM's to do SM conferences? Are there circumstances under which
ASM's (or even the SM) can sit in on the B of R?
Hypothetical situation -- a scout has clearly not "retained" a number of
skills he is supposed to have learned for 2nd class...does not remember
how to orient a map; is not sure of the correct way to sharpen a pocket
knife; is not sure where to start in order to perform rescue breathing...
How does the SM conf and/or B of R handle this??? Should there be a "review"
for the scout before he even sits down for his SM conf?
