The Class I medical form is the parent's update & statement.
A Class II medical is a physical exam within the past 36 months.
Class I & II are required of youth attending camps, except for high
adventure camps, that last longer than 72 hours. Class I & II are also
appropriate for adults attending camps, if the adults are under (as I
recall) age 40.
For adults over age 40, and for everyone attending high adventure
activities, the Class III medical is required.
Class III is a physical exam within the previous 12 months. Same form is
used for the Class II physical.
