The key here is not a form, but a bound log book in which such things are
recorded at the time of the incident in ink. Make a copy of the info for
I had an incident several years ago at summer camp in which the SM and the
father declined to have their son taken to the emergency room when he was
complaining of abdominal pain. On the way home from camp, the boy's
appendix ruptured and he ended up in rather serious condition. The family
came back on the council, claiming that they were negligent. When the
bound health lodge report was presented with the first aider's recommendation
and the SM and fathers responses on two consecutive days, the complaint
was dropped (with apologies to all involved).
