Actually, I'm not sure those forms are from national. I have seen different
variations from different councils. They all contain the same info, just

At least you got a reason :) This has been a topic at our Regional Planning
meetings also. Along with **Lighten Up on the forms and paper work** (we
have 4 different trip permission forms, depending on type and length of trip!).
Our council just doesn't respond. It is almost like they don't understand what
we are asking of them!
For what it is worth, I have Seven Lakes permission slip, and travel permission
slips on Wordperfect 5.0. If someone wants them, I'll be happy to send them.
