I'm not claiming authority here -- I don't have the Class 1 & 2 form in
front of me. However, if memory serves, a Class 1 form should be completed
by _every_ participant (adult & youth) each year for participation in any
part of the program. A Class 2 form (which requires a licensed medical
professional's signature) should be completed every three years for
participants under 40 (the 'should' becomes a 'must' if long-term camping
is involved, I believe). A Class 3 form required for adults over 40 each
year if they are involved in long-term camping. A Class 3 form is also
required of all adults and youth as Don mentioned above.
Personally, I mail out Class 1 forms to all registered leaders and members
each year about this time. Before anyone participates in any activity after
1 June, they must have that Class 1 form completed and returned to me.
Copies of the most recent Class 1, 2, and/or 3 forms for each participant
travel with us on all activities. In five years, I've only needed this
preparation once, and the problem was minor. It was very comforting to know
that I had more information than was needed at the ready. "Yea, we can be
prepared... for what?"
