The only den I remember having a flag for was Webelos. It was
designed by the Den Leader and the boys in the den, and it was a blue
rectangle (about 9" by 12") with the Webelos symbol in the middle, the
den number somewhere on it, and a cord on the bottom for each of the
boys with an overhand knot for each activity pin earned. I don't
think our pack gave out any awards to the dens, but if they did then
we would have attached any ribbons to the flagpole. I believe the BSA
Catalog sells den flags, so you could look there for more ideas (but I
think it's often better to have the flags be something the boys had a
hand in making).
As to general guidelines, personally I would specify:
No larger than 12" square
Tied to a pole between 3 and 5 ft tall
Shows the level of the den (wolf, bear, webelos) and den #
These would just be personal preferences, though, and aren't really
based on much recent experience.
