The card I have (from last year) is white with a red strip on the top and bottom
on the front of the card. At the bottom (in the bottom red strip) it says in
small white text "VOID IF NOT ACCOMPANIED BY A CURRENT BOY SCOUTS OF AMERICA
REGISTRATION CARD". The reverse side contains the Obligation and a place for
it to be signed.
Hope that helps. I think there might be different cards, but I'm not certain.
The number on the card (I guess it is the National supply number) is 5008A.
P.S. As I said in the first post, and as other people seconded, to be an
active member of the OA, you must have a current registration with the BSA in
a position other than merit badge counselor. (Usually registered with a unit,
but also OK if it is registered at the district/council/regional/national
The national policy is that in order to attend an OA function, you must be
currently registered in the lodge. To be registered in the lodge, you must
be an official member of the BSA (and paid up there as well I think). That
is specifically for liability and insurance purposes, but it also helps to
keep up with the protection policy of the BSA and with the commitment to the
unit from the OA.
